Background Check Form

Any adult that will be volunteering their time with the KM Youth Fishing Team will need to have a background check completed by the MN Bureau of Criminal Apprehension. This form must be filled out and in ink, signed and turned into Board of Directors. If you have questions regarding this form please contact Tony at info@kmyouthfishingteam.org
